Why we follow up every patient at home

Most chronic care fails between clinic visits. Here is what our health workers do about it.

A diagnosis at a camp is the easy part. The hard part is the next six months: refilling the prescription, coming back for a test, not stopping when the symptoms ease.

That is why every chronic patient we diagnose is assigned a community health worker who visits at home every month.
